Viva Health Behavioral Health is seeking a highly skilled Community Health Worker (CHW) to provide comprehensive rehabilitative services and case management. The CHW is responsible for the delivery of evidence-based skills training and the cultivation of natural support systems, empowering patients to live as independently as possible. This role is foundational to our clinic’s mission of bridging the gap between clinical therapy and successful community integration.
Core Responsibilities & Clinical Scope
1. Comprehensive Skills Training
- Facilitate individualized and group-based training in essential domains, including Life Skills, Coping Strategies, and Independent Living Skills.
- Provide targeted education on Vocational & Employment Readiness, Educational Advancement, and Personal Management.
- Support medication adherence through Medication Management Education, ensuring patients understand their treatment protocols as part of their holistic wellness plan.
- Assist in practical environmental stability, including Housing Navigation and Home Maintenance skills.
2. Case Management & Community Liaison
- Act as a primary navigator for patients accessing community resources, developing and maintaining strong working relationships with external agencies.
- Work collaboratively with an interdisciplinary care team (internal and external providers) to ensure a seamless continuum of care.
- Maintain clinical flexibility to respond to evolving patient needs and crisis stabilization as required.
3. Treatment Planning & Goal Alignment
- Participate in the assessment process to identify specific patient needs and contribute to the development of the Individualized Treatment Plan.
- Implement specific objectives and interventions designed to help patients build "natural support systems" within their own communities.
4. Administrative Excellence & Compliance
- Maintain rigorous documentation of all service attempts, interventions, and patient no-shows.
- Ensure all clinical notes are authenticated with appropriate credentials, dated, and submitted within established agency and state policy timelines.
- Uphold the highest standards of patient satisfaction and community-partner relations.
Job Types: Full-time, Part-time
Pay: $17.50 - $22.00 per hour
Expected hours: No less than 30 per week
