- Complete occupational therapy evaluations and treatment visits in patient homes
- Address ADLs, home safety concerns, adaptive equipment needs, and fall prevention
- Develop individualized treatment plans focused on meaningful patient goals

- Help patients maximize their independence and confidence
Collaborate With A Great Team
- Partner with nurses, Physical Therapists, Speech Language Pathologists, physicians, and interdisciplinary clinicians
- Contribute to exceptional patient outcomes and continuity of care
- Complete timely documentation and care planning activities
- Serve as a trusted resource for patients and families
Required Qualifications
- Active Minnesota Occupational Therapist (OTR/L) license
- Strong assessment, treatment planning, and patient education skills
- Ability to work independently and manage a visit-based schedule
- Reliable transportation, valid driver's license, and current auto insurance
Preferred Qualifications
- Home health experience
- Experience with geriatric and medically complex patients
- Passion for relationship-based, patient-centered care
