About Monitor My Health
Monitor My Health is a 501(c)(3) nonprofit organization dedicated to improving the health and well-being of individuals through evidence-based health promotion programs. Our initiatives focus on diabetes prevention and management, healthy aging, chronic disease prevention, and fall prevention.
As part of our continued commitment to expanding access to care, we have launched a Mobile Clinic that provides free A1c (blood sugar) testing, blood pressure screenings, and Social Determinants of Health (SDoH) assessments. By bringing services directly into the community, we reduce barriers to care and connect individuals with essential health and social resources.
Position Summary
We are seeking an experienced, compassionate, and motivated Social Determinants of Health (SDoH) Care Navigator to join our growing team. This part-time, hybrid position offers a flexible work schedule beginning at 15–20 hours per week, with the opportunity to grow into a full-time role.
The Care Navigator will serve as a vital link between our mobile clinic participants and community-based services. This individual will help with the logistics of running the clinic as well as assess client’s social needs, coordinate client referrals, advocate for clients, and provide follow-up support to help ensure successful connections to healthcare and community resources.
Key Responsibilities
- Conduct Social Determinants of Health (SDoH) assessments and identify unmet social needs.
- Connect clients with appropriate community resources, healthcare providers, and social service agencies.
- Coordinate referrals and monitor follow-through to ensure clients receive needed services.
- Provide patient advocacy, education, and ongoing support throughout the referral process.
- Collaborate with healthcare providers, community organizations, and Mobile Clinic staff.
- Maintain accurate documentation of client interactions and referrals.
- Protect client confidentiality and comply with HIPAA and organizational policies.
- Assist with data collection and reporting to support program evaluation and grant requirements.
- In partnership with the program coordinator, oversee clinic logistics including scheduling, staffing, and supplies.
- Participate in outreach activities and represent Monitor My Health at clinic events.
Qualifications/ Required
- Bilingual (English/Spanish).
- Bachelor's degree in Social Work, Human Services, Public Health, Psychology, or a related field.
- Minimum of two (2) years of experience in patient advocacy, care coordination, case management, community health, or a related field.
- Demonstrated knowledge of Social Determinants of Health and community resource navigation.
- Strong interpersonal, communication, and organizational skills.
- Ability to work independently while collaborating effectively with a multidisciplinary team.
- Proficiency with electronic documentation and Microsoft Office applications.
- At least 25 years of age (required by insurance) and willingness to drive a Mercedes-Benz Sprinter Van.
