Job Summary
Provides direct care to patients and serves as a consistent care coordinator through the continuum of care, assessing the physical, psychological, spiritual and social needs of the patient. Directs patients to the next appropriate service and tracks their progress. In collaboration with the Breast Center Medical Director, provides health education, promotes wellness and performs a health assessment and physical examination of the breasts.
Essential Functions
- 1. Assess to identify high risk patients (physical, psychological and social) sharing with the multidisciplinary team to collaboratively design a care plan with desired outcomes.
2. Plan healthcare pathways with interdisciplinary care team and intervene when necessary.
3. Coordinate and monitor the patient and triage for needed services across all service lines.
4. Provide education, i.e. disease process, treatments, self care skills etc. and support for patient and family.
5. Monitor patient’s total response to care for optimum outcomes.
6. Performs Intake Assessments, Clinical Breast Exams (CBE), pelvic exams and PAP testing for patients referred through the Rockland County Cancer Services Partnership (RCCSP)
7. Tracks, receives and reports biopsy results to Medical Director (RCCSP) and provides post biopsy education for patients.
8. Performs, tracks, receives and facilitates reporting biopsy results to Physicians.
9. Directs, monitors and supervises all clinical aspects of the Breast Center including pre and post biopsy education for patients.
10. Assists the Administrative Director in preparation of the hospital required quarterly Performance Improvement reports. Participates in the development of new performance goals and their implementation.
11. Supports Breast Center growth by participating in community outreach programs through seminar presentation, programs for Breast Cancer Awareness month in October and informing other primary health care professionals of the services offered at the Breast Center for their patients.

- 1. Within the scope of practice, assumes responsibility, as necessary, for the assessment,
- treatment and care of patients of all ages, from neonate to geriatric.
- 2. Displays competency in obtaining and interpreting the patient’s behavior based upon an
- understanding of cognitive, biopsychosocial, and aging processes.
3. Displays an understanding of the developmental needs of the patient when planning and implementing treatment or services.

Qualifications
- Masters Required
- 1-3 years Work experience as an RN or Nurse Practitioner in Family Medicine preferred. Preferred
NP_NURSE PRACTITIONER - NEW YORK STATE EDUCATION DEPARTMENT NYS NP License Required
