Responsibilities
- Lead and supervise nursing staff to deliver high-quality patient care aligned with facility policies and standards.
- Coordinate patient assessments, vital signs, and medical documentation with precision and attention to detail.
- Administer medications, as needed.
- Oversee clinical procedures such as catheter care, and sterile techniques.
- Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information and accurate medical records.
- Collaborate with interdisciplinary teams on discharge planning, case management, and specialized care such as dementia or Alzheimer's care.
- Utilize EMR (Electronic Medical Record) systems like Epic or Cerner for documentation, order entry, and reporting purposes.
- Support infection control practices through adherence to aseptic techniques and proper specimen collection/processing.
- Provide education on health coaching topics including geriatrics, pain management, nutrition, and chronic disease management.Skills
- Strong knowledge of medical terminology, physiology, anatomy.
- Skilled in using EMR/EHR systems.
- Demonstrated ability to perform advanced procedures such as catheterization, and sterile processing.
- Experience in diverse settings including long-term care facilities like nursing homes or assisted living communities; home & community care; outpatient clinics; or hospital medicine units.
- Familiarity with specialized areas such as hospice care.
- Excellent patient assessment skills combined with the ability to work effectively with individuals with disabilities or developmental challenges.
- Knowledge of HIPAA compliance standards along with strong medical documentation practices ensuring accuracy and confidentiality.
