Chrysalis Home Healthcare is seeking a Texas-licensed physician to serve as a contracted Medical Director / Physician Advisor for our established Medicare-certified home health agency serving Southeast Texas.
This is an excellent opportunity for a Hospitalist, Internal Medicine, Family Medicine, or Geriatric physician interested in a flexible clinical leadership role outside of traditional practice.
About the Opportunity:
Chrysalis Home Healthcare is looking for a physician partner who is passionate about improving patient outcomes, supporting safe transitions from hospital to home, and helping patients remain independent in their home environment.
Responsibilities include:
- Provide physician leadership and clinical oversight
- Collaborate with our nursing and therapy teams
- Support quality initiatives and patient outcomes
- Provide guidance related to home health standards of care
- Participate in clinical discussions as needed
Ideal Candidate:
- Texas licensed MD or DO
- Hospitalist, Internal Medicine, Family Practice, or Geriatric background preferred
- Understanding of medically complex patients and chronic disease management
- Interest in improving care transitions and reducing hospital readmissions
