Legacy Concierge is seeking a dedicated and compassionate Private Duty Registered Nurse I to deliver exceptional quality 1:1 care to patients in the comfort of their homes. This role is designed specifically for RNs who prefer direct patient care with the support of structured mentorship and a clearly defined clinical career pathway. Your focus will be delivering hands-on clinical care and fostering continuity and trust with your assigned patient during your shift.
Nurses in this role work under clinical supervision and mentorship, and are expected to complete structured onboarding, demonstrate patient-centered care, and build competence working in a 1:1 private home environment. All key responsibilities are contingent upon the patient's individualized plan of care

Key Responsibilities
Clinical Care & Patient Management
Deliver 1:1 skilled nursing care in the patient's home.
Perform comprehensive head-to-toe assessments including but not limited to: evaluation of physical attributes, neurological status, respiratory function, cardiovascular status, skin integrity, and musculoskeletal function.
Monitor, administer, and document vital signs throughout the shift, including blood pressure, pulse, respiration, temperature, and oxygen saturation.
Administer medications safely and accurately, including insulin management, blood pressure medications, oxygen, and other prescribed treatments, in accordance with the patient's plan of care.
Support activities of daily living (ADLs) and instrumental activities of daily living (IADLs) including but not limited to: toileting, dressing, feeding, peri care, meal preparation, transportation, and companion care.
Perform G/J-tube feeding and medication administration as ordered.
Provide condition-specific management as directed by the plan of care.
Perform basic wound care and dressing changes.
Communication & Documentation
Complete daily clinical notes accurately and in a timely manner using the agency's document systems (QAPI Plus, WellSky, RN Pad, etc.).
Communicate promptly and consistently with the RN Care Ambassador when there is a change in patient status, an incident, or a clinical concern requiring escalation.
Communicate with providers as directed, including conveying patient status updates, medication changes, and care plan modifications.
Maintain confidentiality of all patients and company information at all times, in full compliance with HIPAA, infection control standards, and internal protocols.
Report incidents or clinical changes promptly to the RN Care Ambassador.
Provide emotional and physical support to the patient and their family throughout the shift.
Team & Operational Responsibilities
Support case management and patient intake responsibilities as needed and as directed by the RN Care Ambassador.
Maintain a reliable schedule, arrive on time, and communicate any scheduling changes according to company policy.
Attend all required training, in-services, and competency evaluations.

Complete structured onboarding and orientation, including shadow shifts prior to independent assignment.
Required Qualifications
Active California Registered Nurse (RN) License in good standing
Graduate of an accredited ADN or BSN nursing program
ADN graduates – minimum of 2 years of hospital experience required; preferably in an emergency room, urgent care, bed side, telemetry, step down unit
BSN graduates - minimum of 8-12 months of hospital or skilled nursing facility experience required; preferably in an emergency room, urgent care, bedside, telemetry, step down unit
Current BLS/CPR certification
Demonstrated ability to work competently in a 1:1 private care environment.
Ability to work independently and follow instructions, ask questions when clarification is needed.
Strong communication, documentation, professionalism, and customer service skills.
Physical Requirements
Ability to stand, sit, kneel, and move for extended periods of time.
Ability to lift, transfer, or reposition patients with appropriate assistive techniques.
Ability to lift or carry equipment and supplies up to 50 pounds.
Use of clinical and technology equipment including tablets, phones, and navigating through EMR platforms.
Visual acuity to observe and assess patient conditions throughout shift.
