Job Goal:
The Special Education Work Experience and Transition Coordinator Teacher is responsible for planning, implementing, and coordinating individualized transition services for young adult students with disabilities ages 14-21. The mission of this role is to transition students from traditional high school academics and develop individual pathways to practical independence, employment readiness, community integration, and self-advocacy. This professional acts as an educator, a case manager, and a community liaison, connecting families with adult service agencies and local employers.
Qualifications:

Wisconsin Department of Public Instruction special education certification, number 1810, 1811, 1801 or 1830 in the high school setting

Demonstrated strengths in positive human relations, cultural competence, problem-solving, and conflict management

Communicate clearly, professionally, and positively with students, staff, parents, and community members
Position Responsibilities:

Coordinates and facilitates transition-focused meetings with students and families to review resources, timelines, and next steps; provides ongoing support and oversight throughout the referral process and while services remain active.

Collaborates with a variety of stakeholders, including educators, employers, agencies, and families.

Collaborates with building principals, directors, and other key stakeholders to ensure the continuum of transition services and programming are in compliance with state and federal transition requirements and align with district initiatives
Reports To: PHS Principal/Associate Principal and/or Director/Assistant Director of Student Services
