Job Overview
FE Medical Center is seeking an experienced, compassionate, and highly responsive On-Call Nurse Practitioner (1099 Contractor) to provide home visits, patient follow-up, transitional care, and after-hours clinical support throughout the Miami-Dade area.
This role plays a critical part in helping patients remain stable after hospital discharge, reducing avoidable emergency room visits and hospital readmissions, and ensuring timely clinical intervention when health concerns arise outside of regular office hours- The ideal candidate is patient-centered, clinically strong, highly organized, and comfortable managing ongoing follow-up for high-risk patients while responding to urgent clinical needs during assigned on-call periods.
Compensation
Compensation ranges from $35 to $65 per hour, depending on experience, availability, qualifications, and scope of services provided.
Key Responsibilities
- Respond to patient calls and urgent clinical concerns during assigned on-call hours.
- Conduct home visits, clinical assessments, and follow-up evaluations as needed.
- Monitor assigned patients based on clinical risk level and care management protocols.
- Maintain regular follow-up communication with patients, caregivers, and families to support treatment adherence and early identification of health concerns.
- Support transitional care management for recently discharged patients and assist in preventing avoidable hospital readmissions.
- Identify changes in patient condition and coordinate appropriate interventions with physicians, specialists, caregivers, and other healthcare providers.
- Perform comprehensive patient assessments, including physical examinations, vital signs review, health history evaluation, and clinical documentation.
- Educate patients and families regarding medications, chronic disease management, symptom monitoring, nutrition, discharge instructions, and home safety.
- Administer medications, injections, IV therapies, wound care, catheter care, ostomy care, and other clinical interventions within scope of practice.
- Utilize EMR/EHR systems to document patient interactions, assessments, interventions, and follow-up activities accurately and timely.
- Maintain accurate records of patient outreach, follow-up efforts, care coordination activities, and clinical outcomes.
- Coordinate care transitions and communicate effectively with hospitals, physicians, specialists, caregivers, and interdisciplinary healthcare teams.
- Maintain compliance with HIPAA regulations, infection prevention standards, and all applicable clinical and operational protocols.
- Deliver exceptional patient service with empathy, professionalism, responsiveness, and strong communication skills.
Required Qualifications
- Active Nurse Practitioner license in the State of Florida.
- Minimum of three (3) years of clinical experience in primary care, home health, hospital medicine, post-acute care, transitional care management, chronic care management, emergency medicine, hospice, palliative care, or related settings.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Experience managing patients with chronic and complex medical conditions.
- Proficiency with EMR/EHR systems.
- Excellent communication, patient education, and relationship-building skills.
- Ability to work independently, prioritize effectively, and manage multiple patient needs simultaneously.
- Availability to respond during assigned on-call periods, including evenings, weekends, and holidays as needed.
- Valid Florida driver’s license and reliable transportation.
- Bilingual English and Spanish preferred.
