Aloha Nursing Rehab Centre is a long-term care facility in Kaneohe, HI that operates 24 hours a day/7 days a week. We are seeking a compassionate, organized, and detail-oriented Social Services Assistant to join our team. In this role, you will provide administrative and program support to the Social Services Department, assisting with discharge planning, resident services, regulatory compliance, and coordination of community resources. You will work closely with residents, families, interdisciplinary team members, and community providers to help ensure successful transitions of care and quality resident outcomes.
Part-Time Schedule: Monday to Friday, approximately 9:00 AM to 1:00 PM. Hours are flexible and can be adjusted as needed.
Essential Duties & Responsibilities
- Assist Social Workers with resident discharge planning and follow-up activities.
- Coordinate post-discharge services, including home health care, transportation, and durable medical equipment (DME).
- Maintain and update community resource databases, including care homes and home health agencies.
- Audit PASRR/EPASRR documentation for accuracy and regulatory compliance under the direction of the Social Services Manager.
- Review discharge checklists and ensure completion of required discharge paperwork.
- Conduct discharge follow-up calls and prepare monthly reports of findings.
- Monitor resident Medicaid renewal dates and provide reminders to families.
- Assist with development of Behavior Care Plans under the direction of the Social Services team.
- Conduct Advance Directive and POLST audits.
- Facilitate daily stand-up meetings and participate in Medicare Part A, Rehab, and Social Services meetings.
- Schedule and coordinate ICF quarterly and annual meetings.
- Provide information regarding facility services to residents, families, and community partners.
- Assist residents with personal shopping needs when family members are unavailable.
- Maintain transfer and discharge notifications in accordance with applicable regulations.
Qualifications
Education
- High School Diploma or equivalent required.
Experience
- Previous experience in a long-term care facility preferred.
Knowledge, Skills, & Abilities
- Strong verbal and written communication skills.
- Excellent organizational and time-management abilities.
- Ability to exercise sound judgment and seek guidance when appropriate.
- Understanding of the social and psychological needs of older adults and other resident populations.
- Ability to maintain confidentiality and professionalism.
- Proficiency with standard computer applications and electronic documentation systems.
