All Anchor Healthcare nurses are fully licensed, CPR certified, and experienced in providing in‑home care to both pediatric and adult patients with complex medical needs.
Position Overview
We are seeking a dedicated, compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing team of Home Healthcare Nurses. This role is ideal for clinicians who value one‑on‑one patient care, continuity, and the opportunity to make a meaningful impact in the lives of medically fragile individuals and their families.
Key Responsibilities
- Provide skilled nursing care to pediatric and/or adult patients in their homes according to individualized care plans
- Perform assessments, monitor patient conditions, and document clinical findings accurately
- Administer medications, treatments, and therapies as prescribed
- Manage and operate medical equipment commonly used in home care settings
- Collaborate with physicians, care coordinators, and family members to ensure high‑quality, patient‑centered care
- Maintain compliance with Joint Commission standards and agency policies
- Promote a safe, supportive, and therapeutic environment for patients and families
Qualifications
- Active RN or LPN license in good standing
- Current CPR certification
- Experience in home healthcare, pediatrics, or adult skilled nursing preferred
- Strong clinical judgment and ability to work independently
- Compassionate, patient‑focused approach to care
- Reliable transportation and ability to travel to patient homes
