POSITION OVERVIEW
This position provides direct case management services to youth and families and facilitates a structured mental health therapeutic group within an after-school program setting.
This is a minimum 15-hour per week part-time role, with opportunities for increased hours based on performance and program needs. Productivity-based bonuses reward initiative, engagement, and results.

WHAT SUCCESS LOOKS LIKE
- Youth are engaged and participating in therapeutic group sessions
- Families feel supported and connected to resources
- Documentation is accurate and submitted on time
- Cases are managed proactively, not reactively
- Productivity goals are met or exceeded
- The Case Manager contributes ideas for program growth and improvement
KEY RESPONSIBILITIES
- Provide individualized case management services to youth and families
- Conduct assessments and develop service plans
- Connect families to appropriate community resources
- Facilitate engaging, structured mental health therapeutic group sessions
- Maintain timely and accurate documentation
- Communicate regularly with families and program staff
- Track progress toward goals and adjust plans as needed
- Meet productivity standards tied to bonus opportunities
- Participate in supervision, training, and professional development

MINIMUM QUALIFICATIONS
- 3 years of case management experience OR a Bachelor’s degree i
- Reliable transportation and personal vehicle required
- Ability to work a minimum of 15 hours per week
- Ability to pass required background screenings
PREFERRED EXPERIENCE
- Experience working with youth or school-based programs
- Group facilitation experience
- Knowledge of behavioral health systems and community resources
- Experience working in fast-paced or community-based environments
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field
