You'll work independently throughout your territory while partnering closely with a supervising Physical Therapist and interdisciplinary clinical team.

Spend Your Day Doing What PTAs Do Best
Your role isn't about sitting behind a desk or spending the entire day inside the same building.
You'll be out in the community helping patients become stronger and more capable in the environments where they actually live.
One patient may be working toward safely navigating the stairs again.
Another may be rebuilding strength after a hospitalization.
Another may need balance and gait training to reduce their risk of falling.
You'll get to provide hands-on treatment, encourage progress, educate families, and see patients accomplish goals that directly affect their ability to remain independent.
What You'll Do
- Provide physical therapy treatment visits in patient homes.
- Implement plans of care established by the supervising Physical Therapist.
- Provide strengthening, balance, gait, mobility, and functional training.
- Reinforce home exercise programs and safety strategies.
- Educate patients, families, and caregivers.
- Monitor patient progress and communicate changes to the supervising PT.
- Complete accurate and timely EMR documentation.
- Collaborate with nurses, therapists, physicians, and other care team members.
Your Experience Can Come From More Than Home Health
We welcome PTAs with backgrounds in Home Health, Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Orthopedics, Senior Living, Long-Term Care, and other rehabilitation environments.
Home health experience is preferred, but the right PTA doesn't necessarily need previous field experience.
We're looking for someone who is organized, dependable, clinically confident, comfortable working independently, and able to build strong relationships with patients.
Qualifications
- Current Minnesota Physical Therapist Assistant (PTA) license.
- Valid driver's license and reliable transportation.
- Strong patient communication and engagement skills.
- Ability to independently manage a field-based schedule.
- Strong collaboration with the supervising Physical Therapist.
- Availability for at least 2 weekdays / 12 visits per week.
- Home health experience preferred but not required.
