Role:
This fully benefited position requires the person to research, secure, protect, and maintain the Estates of the Wards/Clients of Tri-County CAP, Inc./Guardianship Services in a prudent and responsible manner pursuant to Probate Practices and Program Policies and Procedures. To secure necessary services to meet the ward's needs. To make informed decision of behalf of the ward.
Experience: One year to three years of similar or related experience.
Education: (1) A college degree (non-technical discipline) or (2) multi-craft skills acquired through extensive on-the-job training or apprenticeship programs or (3) in-depth industry and job specific technical skills acquired through a combination of formal instruction and on-the job training.
Must be able to obtain National Certified Guardian status through the Center of Guardianship Certification and be a member of the National Guardianship Association within three years of employment with this agency unless otherwise designated and retain this status by meeting the necessary continual training requirements.
Essential Functions & Responsibilities:
- Meet with wards and treatment providers on a quarterly basis or as necessary. Gather information in a face-to-face manner so that informed decisions can be made on behalf of the ward. Attend court hearings, provide testimony, and act in the Pro Se capacity on behalf of the guardianship program as necessary and assigned. Meet with counsel on behalf of ward as necessary and assigned.
- Communicate with wards and treatment providers to ascertain the status of the ward and to make informed decisions as necessary on behalf of the ward. Communicate with legal representatives on behalf of the ward. Communicate with external agencies and treatment providers to secure necessary services on behalf of ward. Such activities are not face to face contacts.
- Travel to and from meetings, court hearings, etc.
- Management of documents pertaining to wards treatment and services including treatment plans, consents and authorizations, reviewing records for information necessary to secure services or otherwise manage the needs of the ward. Maintaining files consistent with Program policy and procedures.
- Engage in activities that are necessary for the operation of the program but are not necessarily specific to a particular ward including attending staff meetings, training, and quality assurance activities.
This position includes remote, travel and onsite at our Ashland office. The successful candidate will cover either the Manchester/Nashua area or Hillsborough area.
