Marshmallow’s HOPE is seeking a compassionate, dependable, and experienced Intensive Home-Based Clinician to provide behavioral health services to youth and families in their homes and communities. This position focuses on assessment, treatment planning, clinical intervention, family stabilization, safety planning, and care coordination.

Position Summary
The IHB Clinician provides timely, trauma-informed, strengths-based, and medically necessary clinical services to youth and families. Services may be delivered in clients’ homes, schools, community settings, Marshmallow’s HOPE offices, or through approved telehealth platforms.
The clinician completes clinical assessments, develops measurable treatment plans, provides intensive interventions, coordinates care, manages safety concerns, and maintains accurate documentation. This position may also provide clinical guidance to assigned IHB Support staff and other members of the client’s care team.
Essential Duties and Responsibilities
- Complete clinical assessments, required screening tools, clinical updates, and evaluations of risk and protective factors.
- Establish and document medical necessity within organizational and payer timelines.
- Develop and update measurable treatment plans based on the client’s needs, strengths, risks, priorities, and service authorization.
- Provide individual, family, and caregiver-focused clinical interventions.
- Help youth and families strengthen coping, emotional regulation, communication, safety, and overall functioning.
- Provide trauma-informed, strengths-based, and client-centered services in real-life settings.
- Develop, implement, and update safety and crisis-response plans.
- Assess and respond appropriately to suicide risk, self-harm, threats, abuse, neglect, exploitation, and other safety concerns.
- Fulfill all mandated-reporting and crisis-escalation responsibilities.
- Coordinate services with families, IHB Support staff, mentors, Family Peer Support staff, schools, healthcare providers, supervisors, and other systems of care.
- Communicate client progress, barriers, risks, and service needs to appropriate care-team members.
- Provide clinical direction and guidance to assigned IHB Support staff when applicable.
- Complete accurate and timely progress notes, assessments, treatment plans, reviews, safety plans, discharge documentation, and required corrections.
- Ensure documentation supports medical necessity, billing requirements, and payer standards.
- Maintain professional boundaries, confidentiality, ethical conduct, supervision requirements, and applicable licensing standards.
- Participate in required supervision, team meetings, training, case consultation, and performance reviews.
- Perform other related duties consistent with the position’s qualifications and scope.
Required Qualifications
- Master’s degree in social work, counseling, psychology, marriage and family therapy, or a related behavioral health field.
- Current Illinois behavioral health license or eligibility for licensure, as required by the position and applicable payer rules.
- Experience or demonstrated competency in clinical assessment, treatment planning, behavioral health documentation, and risk assessment.
- Ability to work independently in homes, schools, and community-based environments while participating in required supervision.
- Ability to respond appropriately to behavioral health crises and safety concerns.
- Strong written, verbal, organizational, and interpersonal skills.
- Valid driver’s license, reliable transportation, and automobile insurance.
- Ability to travel throughout assigned service areas.
- Ability to work a flexible schedule based on client and program needs.
- Ability to satisfy all applicable background-check, credentialing, and employment requirements.
Preferred Qualifications
- Experience serving children, adolescents, and families.
- Experience with youth suicide prevention, crisis intervention, and high-risk youth.
- Training or experience in trauma-informed care, family systems, CBT, or DBT.
- Experience completing IM+CANS assessments.
- Knowledge of medical necessity, service authorization, and billable documentation requirements.
- Experience developing treatment plans and completing clinical reviews.
- Experience providing clinical guidance to behavioral health support staff.
- Experience collaborating with schools, hospitals, community providers, and systems of care.
Work Environment
This is a field-based clinical position. Services may be provided in homes, schools, community locations, Marshmallow’s HOPE offices, and through approved telehealth platforms. The position requires regular travel, timely documentation, flexible scheduling, care-team communication, supervision, and responsiveness to safety or crisis concerns.
Pay: $55,000.00 - $60,000.00 per year
