Summary
We are seeking a dedicated Nurse Practitioner or PA to join our team in providing exceptional rehabilitative care at U.S. Physiatry.
This role will primarily be seeing acute Rehab consults at Palmetto Hospital. A bilingual provider is highly preferred.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer treatments and therapies as prescribed by physicians.
- Monitor patient progress and adjust care plans accordingly.
- Educate patients and families about treatment options and health management.
- Maintain accurate medical records and documentation.
- Participate in quality improvement initiatives to enhance service delivery.
- Ensure compliance with healthcare regulations and standards.
- Qualifications
- Master's degree in Nursing or related field.
- Current Nurse Practitioner or PA license in the state of practice.
- Strong clinical skills with experience in acute or post-acute care settings.
- Proficiency with electronic medical record systems (e.g., Epic).
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a team environment.
- Experience with telehealth practices is a plus.
- Knowledge of discharge planning processes.
