Responsibilities
- Respond promptly to on-call requests for patient assessments, symptom management, and urgent care needs in home or facility settings
- Conduct comprehensive patient evaluations including vital signs, physiological assessments, and symptom documentation using Electronic Health Records (EHR) systems
- Collaborate effectively with interdisciplinary healthcare professionals such as physicians, social workers, and chaplains to develop and adjust individualized care plans
- Manage patient discharge planning and coordinate post-acute hospice services to ensure seamless transitions of care
- Provide education and emotional support to patients and families about disease progression, medication management, and comfort measures
- Maintain accurate medical records in compliance with HIPAA regulations and hospital policies
- Participate in clinical case management activities including diagnostic evaluation, medication administration, and managing complex symptom control
Skills
- Critical care experience with proficiency in managing acute symptoms and complex patient conditions
- Experience working in Level I or Level II trauma centers, ICU units, or emergency rooms demonstrating strong assessment skills under pressure
- Knowledge of hospital medicine, infectious disease care, geriatrics, behavioral health, and post-acute care environments
- Familiarity with EMR/EHR systems for documentation and patient record management
- Strong understanding of medical terminology, physiology, and health regulation policies including HIPAA compliance
- Skilled in patient service delivery with excellent client communication abilities
- Ability to collaborate seamlessly with healthcare professionals across inpatient and outpatient settings
- Experience in discharge planning, home health services, managed care environments, and utilization management
