Center of Illinois Home Healthcare, Inc. is seeking an experienced Per Diem Occupational Therapist (OT) to join our Medicare-certified, ACHC-accredited home health agency.
We are looking for a compassionate, dependable, and motivated Occupational Therapist to provide skilled therapy services to patients in their homes. The ideal candidate will be committed to delivering exceptional patient care while promoting independence, safety, and improved quality of life through individualized treatment plans.
Responsibilities

Evaluate patients' functional abilities and develop individualized occupational therapy treatment plans.

Provide skilled occupational therapy services in accordance with the physician's orders and the established Plan of Care.

Assess patients' ability to perform activities of daily living (ADLs) and recommend strategies to improve independence.

Educate patients and caregivers on adaptive techniques, home safety, fall prevention, and the use of adaptive equipment.

Collaborate with physicians, nurses, physical therapists, speech therapists, home health aides, and other members of the interdisciplinary team to coordinate patient care.

Monitor patient progress and modify treatment plans as appropriate.

Complete accurate, timely, and compliant documentation in accordance with Medicare, CMS, and agency guidelines.

Participate in case conferences and interdisciplinary team meetings as needed.

Maintain professional standards of practice while ensuring quality patient outcomes.
Qualifications

Active Illinois Occupational Therapist (OT) License required.

Graduate of an accredited Occupational Therapy program.

Minimum of one year of clinical experience; home health experience preferred.

Knowledge of Medicare home health regulations and documentation requirements.

OASIS experience preferred.

Experience with Axxess EMR preferred.

Excellent communication, organization, and time-management skills.

Ability to work independently and manage a flexible patient schedule.

Current CPR certification required.

Valid driver's license, reliable transportation, and current auto insurance required.
