Licensed Practical/Vocational Nurse (LPN/LVN) – Home Health
Job Summary
We’re looking for a skilled and compassionate LPN/LVN to provide in home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high quality home health services.
Responsibilities
Provide nursing care according to the RN developed Plan of Care.
Monitor and document changes in patient condition; report to RN/physician as needed.
Assist with activities of daily living to promote patient independence.
Educate patients and caregivers within LPN/LVN scope of practice.
Maintain accurate EMR documentation.
Participate in case conferences and coordinate care with the home health team.
Follow infection control, HIPAA, and company policies.
Requirements
Current LPN/LVN license in the state of practice.
Graduate of an accredited practical/vocational nursing program.
Minimum 1 year acute care experience preferred.
Current CPR/BLS certification.
Valid driver’s license, reliable insured vehicle.
Strong communication, clinical judgment, and ability to work independently.
Physical Requirements
Ability to lift/transfer patients and perform physical caregiving tasks.
Ability to pass required physical exams and annual PPD testing.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $35 to $40 visit commensurate with experience and depending on type of visit.
