Overview

Duties

- Conduct utilization review and management to evaluate the appropriateness of services, ensuring compliance with insurance requirements and hospital policies.
- Perform intake assessments for new patients, gathering comprehensive medical information to support accurate case management.
- Maintain accurate documentation within electronic health record (EHR) systems while adhering to HIPAA regulations to protect patient privacy.
- Engage in discharge planning activities .
- Conduct ongoing case reviews to monitor patient progress, adjust care plans as needed, and ensure optimal resource utilization.
- Support behavioral health and social work teams by addressing psychosocial factors impacting patient recovery and compliance.
Skills
