Summary
Provide quality case management as part of multidisciplinary treatment team. Serves as a communication liaison between family and treatment team. Develops and implement discharge plans, ensuring housing, transportation, after care arrangements. Participate in treatment planning meetings, communicating patient and family information, placement and aftercare details.
Must be organized, ability to work well with adolescent families, document proficiently & good phone etiquette
Responsibilities
- Conduct thorough assessments of clients' needs and develop individualized discharge plans.
- Provide counseling and support in areas such as addiction and behavioral health.
- Utilize motivational interviewing techniques to empower clients in crisis management.
- Collaborate with healthcare providers and community organizations to ensure comprehensive care.
- Maintain accurate documentation and case records in compliance with regulatory standards.
Job Requirements
Bachelor's Degree in Psychology or related field.
