Sunset West Health is launching a new mobile wound care service, extending our history of excellent patient care beyond the walls of our facilities and into our patients' homes. We're starting focused and hands-on — building a program tailored to real patient needs — with plans to grow as we prove out the model.
Many of our patients are discharged from El Paso hospitals and acute care facilities with wounds that still need attention. This role exists to close that gap: to make sure healing doesn't stop just because a patient goes home.
You'll be joining our Infectious Diseases comprehensive care team, working alongside a physician-led group that values clinical rigor and continuity of care.
What You'll Do
- Conduct in-home and facility-based wound assessments for patients recovering from surgical wounds, diabetic ulcers, pressure injuries, venous/arterial insufficiency wounds, and post-acute infections
- Perform wound debridement, dressing changes, and application of advanced wound care products (including skin substitutes/CTPs) per protocol
- Document thorough, defensible clinical notes supporting medical necessity and treatment progression
- Coordinate care with referring hospitals, SNFs, LTACHs, and the Infectious Diseases team to ensure continuity
- Educate patients and caregivers on wound care, infection prevention, and home management
- Travel to patient homes and post-acute facilities across the El Paso area (mileage/travel supported)
Who We're Looking For
- Active Texas NP license (FNP, AGNP, or ANP) in good standing
- Comfortable working independently in home and facility settings
- **Wound care experience preferred — but not required.** If you're an experienced NP looking for a change of pace, or a newly credentialed NP eager to build a specialty skill set with direct mentorship, we want to hear from you
- Strong clinical documentation habits — this role requires precise, audit-ready notes
- Reliable transportation and willingness to travel within the El Paso metro area
- **Bilingual (English/Spanish) strongly preferred** given the patient population we serve

Pay: $135,000.00 - $150,000.00 per year
