Position Overview
South Chicago Parents and Friends is seeking a compassionate, organized, and motivated Qualified Intellectual Disability Professional (QIDP) to join our Community Day Services team in Dolton, Illinois. The QIDP is responsible for coordinating services, person-centered plans, monitoring program outcomes, and ensuring individuals with intellectual and developmental disabilities receive high-quality supports that promote independence, choice, community inclusion, and personal growth.
This position works closely with families, direct support professionals, community partners, and team members to ensure services meet individual needs and comply with all applicable state and agency regulations.
Essential Responsibilities
- Monitor person-centered service plans and outcomes.
- Coordinate and facilitate team meetings.
- Conduct assessments and identify participant strengths, needs, interests, and preferences.
- Monitor participant progress and document outcomes.
- Ensure compliance with all Illinois Department of Human Services (DHS) and waiver requirements.
- Maintain accurate and timely consumer records and documentation.
- Provide guidance, training, and support to Direct Support Professionals (DSPs).
- Advocate for participant rights, dignity, and self-determination.
- Assist consumers in accessing community resources, volunteer opportunities, and meaningful activities.
- Communicate effectively with families, guardians, case managers, and other stakeholders.
- Participate in quality assurance and program improvement activities.
- Support the development of innovative programming, including vocational, recreational, arts, wellness, and community integration opportunities.
Qualifications
- Bachelor's degree in:
- Human Services
- Social Work
- Psychology
- Sociology
- Special Education
- Rehabilitation Services
- Or a related field
- Meets Illinois QIDP qualifications and requirements.
- Minimum one year of experience working with individuals with intellectual and developmental disabilities preferred.
- Knowledge of person-centered planning and community-based supports.
- Strong organizational, documentation, and communication skills.
- Proficiency with Microsoft Office and electronic documentation systems.
- Valid driver's license and acceptable driving record preferred.
Preferred Skills
- Experience with Community Day Services programs.
- Knowledge of Illinois DHS regulations and Home and Community-Based Services (HCBS) requirements.
- Ability to work collaboratively with diverse teams and stakeholders.
- Strong problem-solving and leadership abilities.
