DON/Admissions Specialist Job Description
Job Overview
We are seeking a dedicated and detail-oriented Assistant Director of Nursing/Admissions Specialist to join our healthcare team. The ideal candidate will play a crucial role in the management of patient care processes, ensuring compliance with regulatory standards and facilitating optimal patient outcomes. This position requires a strong understanding of medical management, admissions, discharge planning, and case management within the facility.
The main responsibility of the Assistant Director of Nursing is to ensure that our facility delivers the highest possible level of patient care and adheres to regulations and best practices regarding sanitation, infection control and patient privacy.
Responsibilities
- Assists the Director of Nursing in managing the nursing team
- Work in conjunction with HR in hiring new nurses and overseeing the onboarding process by training them or assigning them to a trainer
- Creating schedules to ensure that departments have adequate nursing coverage for every shift
- Coaching nurses to improve performance and handling discipline issues
- Monitoring expenses to ensure that the facility operates within budgetary constraints
- Participate in staff meetings to discuss patient status
- Conducting employee evaluations that identify nurses’ strengths and areas of improvement
- Making rounds to assess patient status and observe nurses
- Checking patient records to ensure that they are accurate and up to date
The Admissions Nurse will focus on providing new patients/residents and families with the necessary coordination of services for an optimal and seamless transition into the facility. This role also facilitates the internal admissions process from intake to discharge and is responsible for coordination of all tours and admissions, to ensure the process is thorough and the patient has a smooth admission, with a focus on their individualized clinical needs
Responsibilities
- Conduct assessments and evaluations for patients to determine their needs and develop appropriate care plans
- Collaborate with interdisciplinary teams to ensure comprehensive medical management and effective discharge planning.
- Maintain compliance with HIPAA regulations while managing sensitive patient information
- Monitor inpatient cases and provide case management support to optimize patient outcomes throughout their stay
- Facilitate communication between patients, families, and healthcare providers to ensure understanding of care plans and procedures
Experience
- Proven experience in a healthcare setting, preferably within a long-term care setting
- Strong background in medical management, admissions, discharge planning, and case management practices
- Knowledge of HIPAA regulations and compliance requirements is essential
