The Care Coordinator serves as the primary point of contact for children and families enrolled in Wisconsin's Children's Long-Term Support (CLTS) Waiver Program. This position partners with families to identify strengths, assess needs, develop individualized service plans, coordinate community resources, and monitor services to ensure children receive the supports they need to thrive in their homes and communities.
- Complete comprehensive assessments to identify each child's strengths, needs, and goals.
- Develop and maintain individualized service plans in collaboration with families and providers.
- Coordinate and monitor CLTS waiver services and community resources.
- Facilitate team meetings and maintain regular communication with families and service providers.
- Monitor children's progress toward identified outcomes and adjust service plans as needed.
- Maintain accurate documentation and complete all required reports within state and agency timelines.
- Educate families about available resources, waiver benefits, and community supports.
- Advocate for children and families to ensure access to appropriate services.
- Respond to urgent family needs and assist with crisis planning when necessary.
- Collaborate with schools, healthcare providers, therapists, and community agencies.
- Participate in supervision, staff meetings, training, and ongoing professional development.
- Ensure compliance with Wisconsin CLTS Waiver Program policies and Medicaid regulations.
Required

Associates or Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Special Education, or a related field.
- At least one year of experience working with children with developmental disabilities, physical disabilities, or emotional/behavioral health needs.
- Excellent communication, organization, and problem-solving skills.
- Ability to work independently while managing multiple cases.
- Valid driver's license, reliable transportation, and automobile insurance.
- Proficiency with electronic documentation systems and Microsoft Office.
Preferred

Experience working within Wisconsin's CLTS Waiver Program.
- Knowledge of Medicaid-funded services and community resources.
- Bilingual (Spanish/English) is a plus.
