Position Summary
We’re seeking an LPN who is responsible for ensuring operational quality and improving clinical outcomes by supporting our providers and serving our patients. Both professional and flexible, an LPN should demonstrate care and reinforce patient dignity by helping to establish patient health goals, assisting in care coordination to meet those goals and serving as a teacher, advocate and navigator on behalf of the patient within the context of the care team while completing their nursing duties. An engaging personality and compassionate attitude is preferred.
Principal Duties and Responsibilities (License Dependent)
- Maintain adequate supply of medical, nursing and laboratory supplies, assisting with reordering as needed on a routine basis.
- Escort patients to exam rooms, measure vital signs (including height, weight, blood pressure, pulse, respiratory rate, head circumference and temperature when appropriate), interview them briefly, and document all information in the patient's EMR chart.
- Assist provider in exam rooms, setting up Pap tests, culture materials, or procedure supplies as necessary.
- Report to the provider any significant social or other data obtained during conversation with the patient or his or her family.
- Administer medication/immunizations and perform various procedures in conjunction with the patient's visit as described by the provider; e.g. instructs patient in how to collect midstream urine, change into gown for examination, prepare for GTT or UGI series, etc.
- Perform in-office specimen collection and laboratory tests as needed.
- Help maintain logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, etc.) as directed.
- Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls consistent with training and certification, seeking qualified advice from the provider when necessary.
- Participate in the team based approach to care for all patients. Duties may include assisting clinical director/administration with clinical projects within scope of practice and oversight of and feedback for Medical Assistants related to patient care.

- Ability to communicate clearly and concisely through computer, telephone, and other standard office equipment. Ability to build rapport across race/ethnicity with cultural sensitivity and demonstrate sound judgment under stress, is essential.

- Basic PC/Windows computer proficiency and ability to learn new software concepts required.
- Ability to maintain effective and organized systems to ensure timely patient flow.
- Ability to be flexible and energetic in keeping with the fast pace of a medical office environment.
- Demonstrated ability to perform the following:
- Vital sign measurements.
- Collection of urine, blood, stool and sputum specimens.
- On-site pregnancy, hemoglobin, stool guaiac, urine dipstick, glucometer, and strep tests.
- Setup for office surgical procedures.
- Preparation and maintenance of sterile field.
- Proper disposal of sharps and other contaminated material
- Administer injections and do PPD testing.
- Must be able drive or arrange travel in order to work at any/all His Branches, Inc locations.
- Ability to stand or sit for long periods of time, walking, climbing stairs and moving/lifting up to 40lbs. Including full range of motion and dexterity to ensure safe transfer of patients and equipment.
- Experience in working with low-income, diverse and multilingual populations is highly desired.
Required Knowledge, Licensure/Registration and/or Experience, and Professional Development
- Successfully completed accredited LPN training.
- Current NYS LPN License without any negative actions.
- BLS/CPR certified
- Basic knowledge of medical terminology, computer operation, and EHR use.
- Experience in clinical or hospital sites with one-year ambulatory experience preferred.
- Maintain professional license through ongoing certification requirements.
