PRIMARY DUTIES AND RESPONSIBILITIES:
1. The provision of hands-on personal care.
2. The performance of simple procedures as an extension of therapy or nursing services.
3. Assistance in ambulation or exercises according to the plan of care.
4. Recording pertinent information pertaining to the client’s care and strict adherence to the HHA care plan that is on the client’s chart.
5. Providing hands-on personal care including: Baths, oral hygiene, shampoos, assisting with/changing bed linens, assisting patients with dressing/undressing, skin care to prevent breakdown, assisting patients with toileting activities, keeping patient’s living area clean and orderly, as appropriate.
6. Planning and preparing light meals/snacks according to specific dietary requirements of the client, and if necessary, feeding the client.
7. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
8. Performing simple procedures within the HHA’s scope of practice as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
9. Meeting the safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
10. Encourages and maintains safety standards in the workplace and home.
11. Observing and reporting on the patient’s condition and changes in patient's condition to the Care Manager/clinician, and /or Clinical Services Supervisor.
12. Adhering to the agency’s documentation and care procedures and standards of personal and professional conduct.
13. Consistently demonstrates timely communication with agency staff, supervisors, clients, client family and representatives, etc. Promptly returns phone calls.
14. Participating in in service education programs so as to meet regulatory guidelines.
15. Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
16. Consistently represents all company entities to all internal and external customers with integrity and professionalism.
17. Performs other duties as assigned by management.
QUALIFICATIONS:
- CNA Certification required (*for Certified Nursing Assistant position only).
- At least 18 years of age.
- High School diploma or equivalent.
- Ability to read, write, and follow written instructions, and document care given.
- Self-directing with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail and the impaired.
- Related work experience preferred.
- Must be physically able to perform the duties of the position.
- Must exhibit mature responsible behavior, and understand the need for patient confidentiality.
- Must be able to follow direction and accurately report to the Care Manager/clinician any changes in client's condition.
- Possess and maintains current CPR certification.
- Demonstrates tact, patience and good personal hygiene.
- Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.
- Must be willing to travel in both Genesee and Saginaw counties.
Job Type: Part-time
Pay: $20.00 - $25.00 per hour
