Duties
- Respond promptly to on-call patient needs during weekdays, providing clinical guidance and support via phone or telehealth.
- Conduct comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations.
- Collaborate with interdisciplinary teams to develop and adjust individualized care plans aligned with patient wishes and clinical best practices.
- Manage urgent situations by coordinating emergency responses or hospital transfers when necessary, ensuring seamless communication with providers and families.
- Document all interactions accurately within electronic health records (EHR), maintaining compliance with HIPAA and health regulation policies.
- Educate patients and families on symptom management, medication administration, and end-of-life care options to promote understanding and comfort.
Experience
- Background in hospice or palliative care with familiarity in managing complex symptoms related to infectious diseases or geriatrics.
- Experience with electronic health records (EHR) management and medical records documentation is essential.
