Exciting Homecare Agency is seeking a Nurse Manager - Home Health/Hospice to oversee Clinical Staff and make field visits. Servicing patients in all the Fair Lawn area. You must have a car or personal transportation to make visits.

Responsibilities:
As a Nurse Manager/Field Nurse for Home Health, supporting the mission will include the following responsibilities:
- Coordinate and oversee patient services provided by home health aides.
- Review patients referred for service to determine needs and verify that appropriate personnel are available and can be assigned.
- Complete documentation relative to patient assessments, care plans and supervision of home health aides.
- Assure accurate and timely completion of skill assessments, incident reports and other forms.
- Conduct quarterly record reviews and communicate findings and recommendations to the Director of Nursing and other appropriate agency personnel.
- Supervise home health aides for performance relative to patient care and case management, providing feedback and on-site supervision as required. Provide effective coaching skills to improve functioning and overall agency services.
- Conduct performance evaluations of nursing supervisors.
- Oversee the planning, implementation and evaluation of inservice and staff education programs.
- See that all probationary and annual performance evaluations of staff are completed per policies.
- Assist with the maintenance of patient records, statistics, reports and records for purposes of evaluation and reporting of agency activities.
- Inform the Director of Nursing of patient care and/or staffing issues on a timely basis.
- Report patient issues/concerns to Director of Nursing and follow-up as needed.
- Ensure proper maintenance of patient records as prescribed and in compliance with local, state and federal laws.
- Maintain confidentiality of patient and agency information per HIPAA guidelines.
- In the absence of the Director of Nursing, will become the acting DON and will be vested with authority to act on behalf of the DON.
- Other duties as assigned by the Director of Nursing.
Requirements:
- Current and Valid NJ RN license.
- CPR Certificate.
- Professional Liability Insurance.
- Minimum of one (1) year experience as an RN in acute/ subacute setting, homecare experience is preferred.
- Graduate of an accredited nursing program, Baccalaureate degree in nursing or related field is preferred.
- Valid Driver's License and reliable transportation.
- Willing to travel to all the Fair Lawn counties.
- Computer skills and ability to use agency software system.
Pay: $95,000.00 - $105,000.00 per year
