We are seeking a dedicated and compassionate Case Manager to join our team. Responsibilities
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide crisis intervention and management to clients in urgent situations.
- Offer addiction counseling and support for individuals struggling with substance use disorders.
- Collaborate with healthcare providers, social workers, and community organizations to coordinate care.
- Monitor client progress and adjust service plans as necessary to achieve desired outcomes.
- Facilitate access to behavioral health services, childcare resources, and workforce development programs.
- Mentor clients in developing life skills and coping strategies for improved self-sufficiency.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with regulations.
- Supervise interns or junior staff members as needed, providing guidance and support in case management practices.
Skills
- Proficiency in crisis intervention techniques and crisis management strategies.
- Strong understanding of behavioral health issues and available resources.
- Familiarity with motivational interviewing techniques to engage clients effectively.
- Supervising experience is a plus for overseeing team members or interns.
- Knowledge of Medicare processes related to client care is advantageous.
