CHN Home Health Services - Urgent Hiring for Highly Skilled Physical Therapist FT

Duties and Responsibilities:

Perform initial and ongoing clinical assessments to identify patient needs.

Develop and revise a Plan of Care in consultation with the physician and care team.

Provide therapeutic treatments and evaluate equipment needs.

Evaluate outcomes of care with appropriate discharge planning.

Apply infection control concepts to prevent contamination.

Supervise PTA and HHA as appropriate.

Participate in mandatory continuing education and in-service programs.

Wound care management.
Required Skills/Abilities:

Strong therapy skills based on accepted standards of practice.

Excellent interpersonal skills.

Knowledge of durable medical equipment.

Ability to work a flexible schedule and travel locally.

Reliable transportation and valid auto liability insurance.
Required Education and Experience:

Bachelor of Science in Physical Therapy from an accredited program.

Current Texas State License as a Physical Therapist.

Two years' experience in an acute care or rehabilitation setting.
Physical Requirements:

Must be able to lift and/or move up to 50 pounds.

Frequent talking, standing, and hand dexterity required.

Close vision, color vision, and depth perception abilities needed.
