Resident Case Management:
Initial and periodic resident evaluation.
Orientation of residents and families to the daily routine and follow up to ensure smooth transition
Assisting each resident to maintain family and community ties and to develop new ones.
Encouraging resident participation in facility and community activities.
Establishing linkage with and arranging for services from public and private sources for income, health, mental health, and social services.
Assisting residents in making application for, and maintaining, income entitlements and public benefits.
Assisting the resident in obtaining and maintaining a primary physician or source of medical care choice, who is responsible for the overall management of the individual's health and mental needs.
Assisting the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident's health or mental health including but not limited to:
Assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans.
Assisting residents in grievances and recommendations.
Actively participate in quality assurance meetings.
Provide input and assistance in creating and implementing facility policies.
Provide input and assistance in facilitating staff in-services and education.
Collaborating with hospitals and/or Rehab center case managers to ensure safe admissions
Attending community events to enhance marketing opportunities
Admissions and Marketing:
Complete initial request for information.
Schedule and complete tours.
Provide assistance with pre-admission evaluations and review required medical evaluations.
Complete and review all required admission paperwork.
Upload required documents to the electronic medical record.
Developing relationships with local medical providers, hospital’s, nursing homes to establish a source of referrals
Leadership and Supervision of Staff:
Exercises leadership style that fosters collaboration and achieves consensus and cooperation.
Obtains input from staff via regular formal and informal meetings
Takes timely and appropriate actions to resolve identified issues
Resolves conflicts as they may arise
Initiates change to improve the quality of care within the facility
Documentation:
Reviews pre-admission documentation for compliance.
Maintains clinical records on all residents. Documents actions in an appropriate and timely manner.
Develops tracking system to assure the timely completion of periodic assessments.
Maintains compliance with NYS/DOH regulations.
