Sapphire Hospice is growing and hiring compassionate, dependable PRN Licensed Practical Nurses to support our patients and families across the West Valley. This is a flexible, as-needed role covering triage, on-call, and per-visit case management based on patient needs. Ideal for an experienced hospice LPN who stays calm under pressure and wants meaningful work with schedule flexibility.
What you'll do:
- Provide as-needed coverage during the week, evenings, weekends, and on-call, based on patient and staffing needs
- Triage patient and family calls, assess needs by phone, and escalate to the RN as appropriate
- Provide per-visit case management support under RN direction and the plan of care
- Conduct in-home visits for hands-on care, symptom support, and comfort measures
- Report changes in patient condition promptly to the RN and interdisciplinary team
- Complete accurate, timely clinical documentation
- Support patients and families with compassion during end-of-life care
What we're looking for:
- Active, unrestricted Arizona LPN license
- At least 6 months of hospice experience required
- Strong triage and assessment skills with sound clinical judgment
- Reliable transportation and valid AZ driver's license
- Availability for as-needed shifts across the week, including some evenings, weekends, or on-call
- A calm, solutions-oriented "let's find a way to say yes" approach to patient care
