General Summary/Responsibilities:
The Medical Social Worker is responsible for providing social work services to patients in their homes per physician's plan of care.

Education:
Minimum Level of Education Required: Master's degree
Additional requirements:
Area of study or major: Social Work
Preferred educational qualifications: L.I.S.W or I.S.W
Licensure/Certification/Registration:
- Current Ohio License in Social Work
- CPR Certification
Experience:
Minimum Level of Experience Required: 1-3 years of job related experience
Prior job title or occupational experience: One year experience in a health care setting (hospital, clinic rehabilitation center, home care, etc.)
Knowledge/Skills
- Computer skills preferred
- Must be capable of standing, stooping, bending and lifting up to 50lbs.
