Prosperous Beginnings, LLC is seeking a compassionate, organized, and experienced Care Coordinator to join our Residential Crisis Stabilization Unit (RCSU). This leadership position is responsible for coordinating client care, supervising staff, supporting daily program operations, and ensuring the delivery of high-quality behavioral health services.
Under the supervision of the Clinical Director, the Care Coordinator is responsible for coordinating, planning, organizing, staffing, evaluating, and managing services in accordance with DBHDS regulations and the policies and procedures of Prosperous Beginnings, LLC. The Care Coordinator promotes a safe, recovery-oriented environment while ensuring quality care for clients and their families.

Experience and Qualifications:
This position prefers a Bachelor’s Degree in the Human Services field. Supervisory experience is preferred. Current, valid VA driver’s license. Current CPR/first aid certification. Current TB Test, within the past year. Candidate must provide proof of or satisfactorily complete, upon hire, all DBHDS required training. Under the direction of the Clinical Director, the Care Coordinator is responsible for coordinating, planning, organizing, staffing, evaluating, and managing the service in conjunction with the policies and procedures of Prosperous Beginnings, LLC. Serves as a member of the Management Team and is involved in the operations of the RCSU. Ensures quality services are delivered to clients and their families. Performs all work in accordance with the highest professional ethical standards and in alignment with the Prosperous Beginnings, LLC mission, values, and guiding principles.
Responsibilities
- Participates as an active member in a team approach to the delivery of services to clients.
- Provides supervision and training of staff within her/his program(s). Assists Clinical
- Director in developing an effective team model.
- Coordinates transportation needs for his/her program(s) to include communication with external parties as well as required documentation.
- Participates in staff meetings, community meetings, and other staff and program group activities that support the operation of an effective program.
- Assists in the coordination of client admissions and discharges.
- Functions as a co-facilitator in the treatment teams for his/her service.
- Participates in the development and implementation of service plans, objectives, and methods necessary for providing effective services to individual clients and families.
- Maintains the environment to ensure the safety and health of staff and clients are not compromised.
- Provides on-call support for the program on an as-needed basis.
- Monitors staff’s work to ensure quality and timeliness.
- Coordinates with local community providers to ensure proper connections are made for clients to ensure smooth discharge planning
- Assumes responsibility for own professional development including meeting all mandated standards and staying abreast of developments within the field.
- Assumes on-call responsibility as assigned.
- Performs other duties as assigned.
Required
- Current Qualified Mental Health Professional(QMHP or QMHP-T) certification
- Current, valid Virginia Driver's License
- Current CPR/First Aid Certification
- Current Tuberculosis (TB) test completed within the past 12 months
