Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, including vital signs and overall condition, reporting changes promptly to healthcare professionals
- Administer medications and treatments as prescribed, ensuring adherence to medical guidelines and safety protocols
- Support patients with specialized care needs such as Alzheimer’s disease, dementia, hospice care, or end-of-life care
- Operate Electronic Health Record (EHR) systems to document patient information accurately and maintain medical records in compliance with healthcare regulations
- Assist with light housekeeping tasks like cleaning, laundry, and meal preparation to promote a safe and comfortable environment
- Use assistive devices such as Hoyer lifts or wheelchairs safely during transfers and mobility assistance
Skills
- Strong patient observation skills to detect changes in health or behavior promptly
- Experience working in assisted living facilities, nursing homes, or home healthcare environments
- Knowledge of medical terminology and procedures related to home health services
- Familiarity with EMR (Electronic Medical Records) systems for documentation purposes
- Ability to perform catheter care, medication administration, and vital signs measurement accurately
- Compassionate caregiving skills for working with geriatric patients, individuals with developmental disabilities or disabilities in general
- Understanding of home & community care regulations and HIPAA compliance to ensure patient privacy and safety
- Proficiency in basic life support (BLS), CPR, first aid, and heavy lifting techniques for safe patient handling
- Experience providing memory care such as Alzheimer’s or dementia-specific support in a home or long-term care setting
