As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program in the Primary Care setting, in partnership with the primary care practice health. You will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients, families and caregivers, to ensure the delivery of quality, efficient and cost effective health care services. Minimum Qualifications: Current Hawai‘i RN license. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required. Two (2) years of clinic experience in a Primary Care setting. Experience working within a collaborative interdisciplinary team.
Preferred Qualifications: Certification in Case Management. Experience in care coordination for patients. Care management experience. Experience as participant in continuous quality improvement. Working knowledge of EMR (Electronic Medical Record).
