Position Summary
The RN/LPN provides skilled nursing services to home-bound patients following the individualized plan of care. This position involves performing assessments, delivering clinical treatments, educating patients and families, communicating with providers, and ensuring compliance with all regulatory requirements.

Key Responsibilities
- Provide skilled nursing care such as wound care, medication administration, injections, catheter care, and chronic disease management.
- Complete patient assessments and report changes in condition promptly.
- Collaborate with physicians and the care team to update and implement individualized care plans.
- Educate patients and caregivers on medications, disease processes, and home care procedures.
- Document all care accurately and in a timely manner.
- Participate in case conferences, training, and quality improvement activities.
- Maintain compliance with HIPAA, agency policies, and state/federal regulations.
Additional Responsibilities
- Conduct comprehensive assessments (including OASIS if applicable).
- Develop, review, and revise patient care plans.
- Provide clinical oversight and guidance to LPNs and aides.
Qualifications
- Active Indiana RN license
- 1+ year of clinical experience (home health preferred)
- Knowledge of home health regulations and OASIS is a plus
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication and documentation skills
- Ability to work independently
