Adult Day Health Care center in Chino CA is looking for a full time or part time RN. We are a Community Based Adult Services (CBAS) licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.
Duties included but not limited to:
Monitor participant's condition and assess their needs to provide the best possible care and advice.
Conduct all nursing assessments, monitor health problems of center participants, and create nursing care plans.
Refer program participants to appropriate healthcare providers in the community and serve as a liaison with such organizations.
Serve as a key member of the Multidisciplinary Team among all other departments in the center.
Qualifications:
Valid CA RN license
Bilingual in English and Mandarin/Cantonese is preferred.
Pay: $38.00 - $43.00 per hour
