Position Summary
The Case Manager Supervisor oversees Case Managers, guiding them to assist chronically homeless individuals and families. This position facilitates the coordination of supportive services, which can lead to stable or permanent housing, address health care needs, social services, counseling, and education.
The Case Manager Supervisor will collaborate with the Site Leader, Licensed Clinical Social Worker, and other staff to ensure comprehensive service provision for clients, while also developing resources to meet their needs.
The salary for this position is $58,240 to $82,000, plus benefits depending on experience and qualifications.
Specific Responsibilities
- Develop and assign case plans for all new intakes.
- Hold and document weekly community meetings with clients to discuss shelter rules and updates.
- Conduct bi-weekly meetings with Case Manager staff to ensure they understand shelter grievance policies.
- Attend mandatory shelter grievance committee trainings to ensure compliance.
- Complete critical incident reports as necessary for emergency situations.
- Report personnel concerns to the Site Manager or/and Human Resources within 24 hours of any incident.
- Review Case Managers' timesheets daily to ensure accuracy and compliance.
- Review weekly Case Management Daily Reports and ensure services/needs are met in a timely manner.
- Approve and ensure the timely completion of intake paperwork and case plans.
- Conduct bi-weekly reviews of Case Managers' client case plans to ensure paperwork and clients' needs are addressed.
- Monitor client medical/dental appointments and educational needs.
- Participate in weekly case conferences with Case Managers to discuss client activities and concerns.
- Ensure quality control within the One System and accurate reporting.
- Coordinate and update community resources for clients.
- Attend regular staff meetings, training sessions, and interagency collaboration meetings.
- Communicate effectively with clients, staff, and external partners in both English and Spanish to ensure accessibility of services.
