Job Summary
We are seeking a compassionate and dedicated Social Worker Assistant to join our team. The ideal candidate will possess a strong commitment to helping individuals and families navigate through challenges, providing support and resources to improve their overall well-being.
Duties
- Processes all inquiries in a timely manner and maintains tracking documentation. Meets with family members, or a referring agency, and determines an appropriate program.
- Performs the Food Program enrollment of the client. Collects and certifies the eligibility of the client. Verifies certification and prepare eligibility roster. Totals the meal count summary and transfer meal count to the claim form.
- Provides full case coordination for ADHC participants and comprehensive information and referral resource file for the center’s primary and secondary service areas along with pertinent referrals outside of service area.
- Completely signs and dates quarterly progress notes.
- Provides functional assistance in the areas of assisting participants in any area of their personal care needs, program needs, housekeeping, and maintenance of facility.
- Facilitates or co-facilitates caregiver support groups as assigned.
- Maintains a working knowledge of facility service areas, including transportation, community characteristics and geography.
- Active participant in the ADHC multi-disciplinary team.
- Active participant in all staff meetings and in-services as assigned.
- Works as an integral participant of a management team concept. Maintains effective relationships with staff, geared to the building and maintenance of a cohesive team.
- Consults with and advises staff members as to the relationship of social, emotional, and ethnic factors to health and medical care, and as to the availability of social services in the community.
- Maintains all agencies, licensing, contracts and administrative requirements for documentation as required for client services and administrative needs, including raw data collections and tabulations and reporting requirements.
- Assist in developing and providing educational and training programs for caregivers and professionals. Assist in providing fact sheets and other information materials.
- Acts as an advocate for elderly concerns.
- Participates as a member of assigned community committees as a representative of Antelope Valley Adult Day Health Care Center.
- Maintains a networking liaison with other organizations in the eldercare field. Represents the agency in contact with human service, health care, and community organization groups and individuals.

- Provides in-home assessments for all ADHC participants. Completes social services assessments on all participants.
- Provides group sessions in the program.
- Assist with agency outreach efforts.
- Assist in preparing and giving meals and snacks, if necessary.
Requirements
- A Bachelor’s degree in social service or related fields and receives supervision, consultation, and in-service training from social worker

Work hour: Monday-Friday, 7:30 am - 2:00 pm
Pay: From $19.50 per hour
