About the Opportunity
HomeMD Housecall Services, currently transitioning to HomeNP Health Solutions, is expanding our team of Nurse Practitioners serving the Flint and Genesee County region.
This field-based position allows providers to deliver comprehensive primary care directly to older adults and medically complex patients in their homes, assisted living communities, and other residential care settings. By bringing healthcare to patients, our providers help improve access to quality medical services while reducing unnecessary emergency room visits and hospitalizations.
Providers are assigned patients within a geographic territory designed to minimize travel whenever possible. Most daily driving remains within approximately a 45-mile radius of your home location.
Communities Covered
Flint, Grand Blanc, Burton, Davison, Fenton, Flushing, Swartz Creek, Mount Morris, Clio, Genesee Township, Goodrich, Linden, and surrounding Genesee County communities.
What You'll Do
Deliver Comprehensive Primary Care
- Conduct new patient evaluations and routine follow-up appointments
- Assess acute illnesses and ongoing medical conditions
- Create individualized treatment plans based on patient needs
- Order, review, and interpret laboratory and diagnostic studies
Manage Chronic Medical Conditions
Provide ongoing care for patients with conditions including:
- Diabetes
- Hypertension
- Congestive heart failure
- COPD
- Arthritis
- Dementia
- Other chronic health conditions common among geriatric patients
Adjust care plans as patients' health needs change while emphasizing preventive care and long-term wellness.
Medication Management
- Perform comprehensive medication reviews
- Monitor medication effectiveness and potential interactions
- Coordinate prescription adjustments with collaborating providers and pharmacies
- Educate patients and caregivers regarding medication compliance
Preventive & Wellness Care
- Complete Annual Wellness Visits
- Perform preventive screenings and health risk assessments
- Encourage healthy aging and independence
- Identify opportunities for early intervention
Coordinate Patient Care
- Work closely with physicians, specialists, home health agencies, caregivers, and residential facilities
- Support smooth transitions following hospital or rehabilitation discharges
- Maintain accurate and timely electronic medical documentation
Educate Patients & Caregivers
Provide guidance on:
- Chronic disease management
- Nutrition and healthy lifestyle choices
- Fall prevention
- Mobility and safety
- Treatment plans and healthcare goals

- Fall risk evaluations
- Laboratory and diagnostic test ordering
Team Support
You'll be backed by a collaborative clinical support team that assists with:
- Scheduling

Qualifications
- Current Michigan Nurse Practitioner license
- National Board Certification as a Nurse Practitioner
- Master's or Doctoral degree in Nursing
- Excellent clinical judgment and communication skills
- Ability to work independently in a mobile care environment
Experience in any of the following is preferred:
- Primary Care
- Geriatrics
- Internal Medicine
- Home-Based Primary Care
- Palliative Care
- Family Practice
