I. JOB SUMMARY/RESPONSIBILITIES:
- Responsible for the coordination of patient care throughout the continuum in collaboration with all healthcare providers and the multidisciplinary care team to provide seamless process for oncology patients.
- Serves as an advocate, interpreter, educator and clinical resource with expertise in medical, surgical, and radiation oncology and care management.
- Serves as a liaison throughout the facility and in the community regarding services provided for this unique patient population.
- Coordinates all aspects of the perspective treatment plan regimen.
II. TYPICAL PHYSICAL DEMANDS:
- Essential: finger dexterity, seeing, hearing, speaking, standing, sitting, walking.
- Frequent: pushing/pulling usual weight up to 50 pounds of force, carries usual weight of 10 pounds up to 15 pounds, repetitive arm/hand motions.
- Occasional: stooping/bending, climbing stairs, walking on uneven ground, squatting, twisting body, lifting usual weight of 75 pounds with assistance, reach above, at and below shoulder level.
- Operates physiological monitoring equipment, computer, printer, copy machine and typewriter.
III. TYPICAL WORKING CONDITIONS:
- Not substantially subjected to adverse environmental conditions.
- Work environment is fast paced due to multiple demands.
- May be required to flex hours to meet departmental needs.
- May require off island travel as needed.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
- Current Hawaii State License as a Registered Nurse.
- Bachelor’s degree in Nursing.
- Certification in Oncology Nursing preferred.
- Current certification in BLS/HCP.
B. EXPERIENCE:
- Two (2) years nursing experience, in an outpatient and/or acute care setting.
- Demonstrated ability/skills in patient teaching.
- Demonstrated ability in conducting performance improvement activities.
- Experience with computer based medical records.
Assignment to Head & Neck Institute:
- Two (2) years nursing experience with head and neck oncology patients preferred.
