Job Overview
As a Licensed Clinical, Marriage and Family, or Addictions Social Worker, you will perform psycho-social assessment and referrals for NHC patients/clients when requested. Develop and maintains collaboration with community mental health providers. Maintain patient confidentiality and ensures compliance with HIPAA policies and procedures.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Works as a team member with primary care staff in meeting the behavioral health needs of our patients.
- Provides evidence-based behavioral health assessments and treatment.
- Makes independent decisions regarding needs of the patients, collaborating with primary care staff in providing behavioral health services.
- Maintains required patient records and other written information as required.
- Facilitates the patient receiving other behavioral health services as needed such as Inpatient, Intensive outpatient, and Psychiatric care.
- Provides consultation and information to primary care staff regarding pertinent behavioral health issues.
- Maintains patient confidentiality.
- Works to acquire knowledge of medical issues pertinent to the provision of effective behavioral health services.
- Provides limited in-house counseling (including crisis intervention) for high risk patients in cooperation with other disciplines:
-Evaluates patient’s social and environmental status through chart/record analysis, patient interviews, and consultation with other clinic disciplines.
-Involves client/patient in identifying and understanding problems.
-Evaluates data and suggests treatment/care plan that provides maximum benefit to patient.
-Educates and supports patient regarding options and services available. Provides referrals/interventions and follow up to patients needing assistance from other community agencies.
-Is non-judgmental of lifestyle or cultural differences.
- Maintains departmental policies and procedures and ensures quality of care with respect and sensitivity to the needs of the clients we serve.
- Participates in professional development activities and maintains professional affiliations.
- Complies with all agency policies and procedures concerning corporate compliance including privacy of protected health information.
- Attends and participates in meetings and in-services as required. Participates in professional development activities. Chairs and serves on committees as requested.
- Performs duties in accordance with NHC Standards of Conduct and Mission Statement.
- Performs related work as required and other duties (similar physical requirements and OSHA risk level) as assigned.

- Clinical-demonstrates a working knowledge of the principles and practices of social work (assessment, planning, implementation, and evaluation) and counseling techniques; demonstrates a working knowledge of community resources and referral agencies; maintains knowledge of, and acts in accordance with, current NHC policies and procedures.
- Behavioral Health – Knowledge of and skill in providing behavioral health assessment and treatment specifically in an evidence-based manner. Able to provide information about behavioral health issues to primary care staff and to patients.

REQUIRED QUALIFICATIONS:
Education / Training
Graduate degree in Social Work or Psychology from an accredited institution.
Experience
Prefer a minimum of three years post-graduate experience, in a community based health or social service organization.
Licensure/Certification
Possession of an Indiana State Clinical Social Work License.
Pay: $64,800.00 - $90,700.00 per year
