Duties
- Perform detailed patient assessments, including vital signs, physical examinations, and comprehensive health evaluations.
- Administer medications via IV infusion, injections, and other routes while adhering to aseptic techniques and safety protocols.
- Manage complex wound care procedures such as suturing, ostomy care, and dressing changes with precision and compassion.
- Conduct specialized treatments including botulinum toxin injections, dialysis management, and ventilator support as required.
- Educate patients and their families on disease management, medication administration, nutrition, and health coaching to foster independence.
- Collaborate with interdisciplinary teams to develop personalized care plans aligned with medical documentation standards and HIPAA regulations.
- Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities and progress notes.
Qualifications
- Valid nursing license with current registration in the state of practice.
- Proven experience in home health nursing or related settings such as hospitals, nursing homes, assisted living facilities, or hospice care.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and laboratory procedures.
- Experience working with individuals with developmental disabilities, memory care needs, or behavioral health conditions is highly desirable.
- Competency in critical care procedures including ICU experience, trauma medicine at Level I or II trauma centers preferred.
- Familiarity with EMR/EHR systems such as Epic or Cerner; proficiency in medical documentation and medical records management.
- Ability to perform complex procedures like catheterization, tube feeding management, sterile processing, and infection control practices.
- Excellent communication skills for triage assessments and patient education; ability to work independently while collaborating effectively with healthcare teams.
