Job Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. The LPN will work collaboratively with healthcare professionals to ensure the well-being of patients while adhering to best practices in nursing.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to evaluate patient health status.
- Assist with daily living activities and support patients with personal hygiene needs.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Perform procedures such as IV insertion, catheterization, and wound care as needed.
- Educate patients and families on health management and disease prevention strategies.
- Participate in discharge planning and coordinate follow-up care for patients transitioning from acute settings.
- Ensure a safe environment by adhering to infection control protocols and safety standards.
Experience
Candidates should have experience in one or more of the following areas:
- Critical care or ICU experience
- Long-term care or assisted living environments
- Hospital or emergency room settings
- Working with individuals with developmental disabilities or Alzheimer's care
- Medical office experience, including triage and patient assessment
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks
- Knowledge of ICD coding (ICD-9/ICD-10) and medical terminology
- Experience in geriatrics, pediatrics, or home care settings
The successful candidate will demonstrate proficiency in patient care techniques, possess strong communication skills, and have a commitment to continuous learning in the nursing field.
