Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene
- Monitor patient health by checking vital signs such as blood pressure, pulse, and respiration; observe and report changes in condition
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and compliance
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary
- Support clients with mobility using proper manual handling techniques and patient transfer methods such as Hoyer lifts or other assistive devices
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
- Provide companionship and emotional support, engaging clients in social activities to promote mental well-being
- Document care activities meticulously, update care plans regularly, and communicate effectively with healthcare teams and family members
- Assist with transportation for medical appointments or errands, ensuring client safety during travel
- Follow all protocols related to patient observation, dementia care, Alzheimer's care, memory care, hospice care, autism support, developmental disabilities support, neurodevelopmental disorders assistance, geriatrics, and home health services
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments preferred
- Demonstrated ability to monitor patients’ vital signs and recognize signs of medical emergencies or changes in condition
- Hands-on experience with medication administration and understanding of HIPAA privacy standards
- Knowledge of specialized care such as dementia care, Alzheimer's care, hospice support, or memory care is highly desirable
- Familiarity with patient transfer methods including Hoyer lifts and patient personal care assistance techniques
- Experience working with individuals with disabilities or neurodevelopmental disorders is advantageous
- Strong communication skills for effective client interaction and collaboration with healthcare professionals
- Ability to perform heavy lifting safely when needed for patient transfers or mobility assistance
- Certification in CPR and First Aid is required or obtainable upon hire to ensure readiness for emergency situations
