What You'll Be Doing
- Provide skilled hospice nursing care during evening and overnight hours.
- Deliver pain and symptom management through a comfort-focused approach.
- Assess changing patient conditions and provide appropriate clinical interventions.
- Support patients and families through education, communication, and guidance.
- Provide care in private homes, assisted living communities, memory care settings, and skilled nursing facilities.
- Coordinate closely with physicians, social workers, chaplains, hospice aides, and fellow clinicians.
- Complete timely documentation supporting continuity and quality outcomes.
- Help patients remain comfortable while honoring their goals of care.

Who This Role Is Built For
- Active Minnesota Registered Nurse (RN) License
- Hospice experience preferred but not required.
- Strong consideration given to nurses with experience in ICU, ER, Oncology, Palliative Care, Critical Care, Med Surg, Home Health, or hospital-based nursing.
- Nurses interested in transitioning into hospice are encouraged to apply.
- Strong communication and patient education abilities.
- Excellent clinical judgment and decision-making skills.
- Confidence working independently in the field.
- Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area.
