Position Summary: Community Health Workers (CHWs) are certified, trained public health professionals who enhance healthcare delivery through integrated and coordinated services across the continuum of care. Under the supervision of a medical provider and within a collaborative care model, CHWs at CHA are committed to improving patient outcomes related to social determinants of health, increasing access to community resources, and supporting patients in managing and preventing chronic conditions.

Education and Experience
● Minimum of 2 years of healthcare or community-based experience required
● Must be able to read, write and speak Spanish fluently
● Must have a valid Community Health Worker certificate recognized by the state of Nevada or obtain the certificate within 3 months of hire
● Experience with electronic medical records (EMRs), preferred
Physical Requirements, Office and Clinical Environment Requirements
● Ability to travel between clinics and community sites
● Mobility in clinical and office environments
● Occasional lifting of materials (up to 25 lbs.)
● Clear verbal communication for in-person and phone-based interactions
● Visual ability to read printed and digital information
● Manual dexterity for data entry, paperwork, and handling office tools
● Work occurs in clinical, office, and community settings (e.g., outreach events, patient homes)
