Overview
We are a resident-centered skilled nursing facility dedicated to providing quality care, dignity, and support to the individuals and families we serve. We are seeking a compassionate, knowledgeable, and motivated Licensed Social Worker to join our interdisciplinary team and make a meaningful difference in the lives of our residents.
Duties
- Conduct psychosocial assessments for residents upon admission and as needed.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Coordinate discharge planning and transitions of care.
- Provide counseling, support, and crisis intervention for residents and families.
- Assist residents and families in accessing community resources and financial assistance programs.
- Participate in care conferences and interdisciplinary team meetings.
- Ensure compliance with federal, state, and facility regulations regarding social services.
- Advocate for resident rights and promote resident satisfaction and quality of life.
- Maintain accurate and timely documentation.
Requirements
- Current Idaho Social Work License (LMSW, LSW, or LCSW preferred).
- Bachelor's Degree in Social Work required; Master's Degree preferred.
- Previous experience in skilled nursing, long-term care, healthcare, or geriatric settings preferred.
- Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations.
- Strong communication, organization, and interpersonal skills.
- Ability to work effectively with residents, families, physicians, and interdisciplinary team members.

Pay: $25.00 - $30.00 per hour
